Standard State Application (Form 678)

The State Personnel Board Web site has several versions of the Standard State Application Form 678 available on-line for your convenience. Please remember you MUST follow the instructions on the Exam Bulletin indicating where to submit your application for each exam. Simply filling out an on-line application will not submit your application. You must print your application, sign it and submit it to the testing office listed on the bulletin.

The Standard State Application Form 678 is used to file for most State Examinations. A few departments use customized application forms for some of their examiniations. Please, read the Exam Bulletin carefully to determine what type of application form should be filed for each exam.

The Acrobat Application allows you to fill out the application, print it and save it for future use.

The Online Application (secure database) is a database driven Web application that generates an exact replica of the official State Application without actually filling one out.

Users simply enter fields into multiple Web screens and their data is written to a secure database. When they are ready to print the application, the data is fed into a copy of the form automatically and displayed as an Adobe Acrobat form.
