The official California State Examination/Employment Application Form 678 (Rev. 12/2001) and Instructions are now available electronically on this Web site (at the bottom of this page) in Adobe Acrobat format. Please read the following important notes before attempting to download and use the electronic form.

- The Application Form and Instructions are created in Adobe Acrobat format. In order to use this version of the state application, you will need to DOWNLOAD and INSTALL Adobe Acrobat Reader. The latest version of Acrobat Reader is available free from Adobe's website .
- This version of the State Application supports filling out, printing and saving the form on your computer.
- Or, you can use one of our other state application processes which allows later retrieval and/or revisions to information you have already filled-out. The links for these other forms can be found on the Non State Employee's or Current/Former State Employee's page.
- The California State Personnel Board does not provide support for problems downloading/printing using Adobe Acrobat. We will publish any known printing problems with this form. You should print a test page (first page of the form, partially filled out if you intend to fill out the entire form on your computer and print it), and test that with your printer before spending a lot of time filling out the form and then finding out that you have a printer problem. If you want a blank application form, simply download and print the application without filling anything in.
- Be sure to download and read the instructions before entering your data on the application as it contains valuable information on filling out the form.
- Carefully complete your application and submit it by the final filing date as indicated on the exam and/or vacancy bulletin. Late applications are not accepted.
- At this time, you may not submit the Application Form electronically. You must still print, sign, and send your application per the posting's instructions.
