To process your application and conform to requirements for statistical
information from a number of State and Federal agencies, we need your
Social Security Number, age, ethnic, disability, and gender information.
Transmitting this information on the Internet may cause you some
concerns. We are providing a separate form below for you to use that
is not directly connected to your other personal information (name,
address, etc.). You will also enter a Personal Identification Number
(PIN) below, which you will establish yourself, that will be used to
resolve any examination issues, or for any subsequent
connections you make to the system to update your contact information.
Take the time now to decide on a PIN number. It should be between
six (6) and ten (10) characters in length. You may use any
alphabetic or numeric characters. Punctuation marks or other
non-alphanumeric characters cannot be used.
Providing your Social Security Number is voluntary in accordance
with the Federal privacy act of 1974 (PL-93-579). In this examination, your
Social Security Number is required to check for eligibility to retake the
examination, and verify statements regarding prior employment.
WARNING: You are about to send personal information over the Internet. Precautionary
measures are in place; however, the State Board of Equalization cannot
guarantee the security of the information you are transmitting over the Internet. It might
be possible for unauthorized individuals to gain access to your personal information.
If you do not want to provide the essential information over the Internet, it will be
necessary for you to complete a manual examination process. Regretfully, the manual
examination process
is not as time expedient as the Internet examination process. Therefore, the manual
process can involve delays
in establishing your eligibility for employment. You may
